英文要約TITLE: Research on the Development of Diagnostic Measures and Preventive Medicine for Early Stage Alzheimer’s Disease through a Combination of Brain Imaging, Clinical Research and Information Technology. FY2012 Annual Report
(1) Clinical, Neuropsychology and Pathology cores: The basic design of J-ADNI2 clinical study was finalized, and the study protocol, including schedules for clinical/imaging work up, was created. Forms for explanation to participants and informed consent also were formulated. IRB approval was obtained at the principal investigator’s site (Univ. Tokyo). Creation of procedures manual, and the preparation for the study management and support at the clinical study support center at Univ. Tokyo were initiated. The neuropsychology core examined the test batteries, including Free and Cued Selective Recall Test that was adopted in J-ADNI2, and established the procedures for autopsy cases from J-ADNI1 and 2. (2) MRI core performed standardization of study protocol of acquisition of MRI data using 3T MRI in not only morphological imaging, but also diffusion tensor imaging (DTI), resting-state functional imaging (rs-fMRI), and brain perfusion imaging using arterial spin labeling (ASL). The study protocols for DTI, rs-fMRI, and ASL were determined in the same vendor MRI scanners. (3) PET core: The following preparations were made for the full-scale start of the research from 2013FY. a) The sections related to PET of the J-ADNI 2 protocol were developed. The protocol contains the detailed PET imaging protocol and the risk assessment of radiation exposure. b) The J-ADNI PET QC Core in collaboration with amyloid and FDG PET cores investigated many issues to start PET study smoothly, which were as follows:1) Decision of the PET tracers for amyloid imaging 2) Selection of PET facilities for FDG-PET and amyloid imaging 3) Development of the quality standard of PET tracers for amyloid imaging and 4) Development of the teleradiology system for screening review of amyloid imaging. (4) Biochemical core: Niigata U. found candidate genes along with AD pathological progression (NFT and plaque stage) by exon array analyses in collaboration with the Brain Bank. (5) MRI technology development: Pulse sequence program was performed for Non-contrast MR Perfusion acquisition with Arterial Spin Labeling (ASL). We develop long labeling ASL pulse sequence, as well as vessel signal suppressed ASL pulse sequence. We observed improved SNR and less vessel artifact with this pulse sequence. Phantom for perfusion pulse sequence validation was developed and performed initial test. (6) IT core developed CS Square (Clinical Study Cloud Service) on the VPN network, which manages both clinical data and DICOM data. Especially, DICOM data is strictly anonymized and all of the documents are automatically checked. This system was investigated, and its high performance and usability were validated. Some machine learning methods with ADNI data were evaluated for BigData analysis, and also SS-MIX2 was examined.
