英文要約 The purpose of this research and development is to develop the artificial knee joint which optimized to the bone geometry of the Thais and reconstructs the joint function based on the lifestyle.
Bone geometry of Thais people and the market were investigated in cooperation with the research institute and universities in Thailand. The differences between Japan and Thai people were analyzed during feasibility study in fiscal year 2014.
First we had series of discussions in Thailand for the agreement to collaborate with institute in Thailand to consider the artificial knee joint design for Thai people.
Thai market research was performed to make many Japanese artificial knee joints may be accepted in Thailand. Building relationship with the Thai university and Japanese Embassy has started. It was found that the most popular type of the artificial knee joint in Thai market is PS type, and instruments to operate simply is much in demand to perform an operation in a short time.
Three-dimensional bone models were constructed using CT images of Thai and Japanese people to investigate the bone geometry and measured value were compared. Reproducibility of data also checked.
The target of measurement is female with the normal knees from 50’s to 70’s of the Japanese and the Thai people.
Measurement points are mainly the Antero-Posterior (AP) and Medio-Lateral (ML) of the condyle which are believed to be basic and anatomically important. Anterior bowing of the femur which seemed to vary with the lifestyle and the nation was also measured.
The anterior bowing of Japanese seemed to be larger by only 30 cases of analysis. This may be caused by the lifestyle which Japanese sit straight (Seiza) in daily life since childhood. The Japanese was larger than Thai about the dimension related to AP and ML in a condyle. The bone length is the same mostly in both groups, so the Japanese knee joint is larger for the same height. The aspect ratio (AP/ML) in Japanese condyle is 0.89 and Thai is 0.91, Japanese condyle is flatter than Thai.
Judging from this investigation, there is a possibility that not only European and American artificial knee joint but also Japanese one mismatch in Thai. And a similar figure product shouldn't be offered due to the AP/ML difference.
The meeting with which Japanese and Thai doctors was held for sharing the information and to have a discussion about the market of artificial knee joint in Thai and measuring result of the bone geometry in February. Five doctors familiar with total knee arthroplasty, the bone geometry and knee kinematics participated from Japan, and doctors of universities participated from Thailand. Thai doctors had much interest and showed the intention to cooperate in this research and development. Firm connections could be made in Thai, so smooth development can be expected in the future.
